Job Summary
As a Social Worker, he/she will lead community awareness efforts on Integrated Community Case Management (ICCM) and ensure access to healthcare services. This includes educating communities on disease prevention, facilitating patient referrals, coordinating doctor visits, and supporting follow-up care. The Social Worker will act as a liaison between healthcare providers and the community, improving health-seeking behaviors and reducing barriers to care.
Key Responsibilities
- Facilitate referrals for patients, ensuring they receive timely access to medical services by coordinating with doctors, healthcare providers, and referral networks.
- Organize and oversee community outreach programs, health education sessions, and awareness campaigns, engaging key community members to increase knowledge and adoption of healthy behaviors.
- Support patients and their families in navigating healthcare services, addressing barriers to care such as transportation, finances, and stigma.
- Coordinate doctor visits and mobile clinic schedules, ensuring patients requiring specialized care are attended to in a timely manner.
- Conduct home visits and follow-ups with patients to monitor treatment adherence, provide psychosocial support, and reinforce health messages.
- Establish and maintain strong relationships with local health workers, community leaders, and medical staff to enhance referral systems and community-based interventions.
- Keep detailed records of patient referrals, follow-ups, and community education efforts, ensuring proper documentation and data tracking.
- Advocate for vulnerable populations, including women, children, and individuals with chronic illnesses, to access necessary medical and social services.

Education / Experience Needed
Nursing Bachelor degree (4 year) preferred.
Essential experience working in case management
Desirable 2 years of previous experience in other NGOs
Language
- Kurmanji required
- Arabic desirable
- English desirable
